How to fill out the form online:
1) Click the link above that matches your bargaining unit.
2) Download the file and open it with Adobe Acrobat Reader or Choose “Open with” and select Kami, Lumin PDF, or DocHub. (Do Not choose Google DOC)
4) Click each box where information is needed and type in your response.
5) Save the form, then send the form to GreenfieldEducationAssociation@gmail.com
Alternatively, you can print the form and fill it out by hand. Please send any hard copy forms to Kim Herlihy at GMS.
The updated Dues sheet for 2026-27 will be available at the start of the school year.
